SY22- 23 Immunization Policies and Important Dates

  • DC Health Immunization Forms
  • All students attending public school in DC must provide up-to-date immunization documentation or proof of medical or religious exemption in order to attend school. If the proper immunization documentation is not presented to the school in time, the student may be removed from school. The first date of exclusion for non-compliant PK3-3rd grade students is November 9. The first date of exclusion for non-compliant 4th-8th grade students is December 9.

  • All students age 12 and older must be vaccinated for COVID-19. Official notice will be sent to non-compliant students on November 21; the first date of exclusion for non-compliant students is January 3, 2023.

If your child suffers from asthma and/or allergies, please print and have your doctor* complete the following action plan form(s) and drop them off at the Front Desk:
